Thumb Volar Pulp Loss

Thumb is the most vital structure of all fingers of the hand. About 40% of the activity is carried out by thumb . When thumb is injured his daily activities will be affected not only his own but his family. why pulp is the main in a thumb is it has sensation that is needed to maintain power and fine grip for holding any object or key for that matter



Case report

This case a 25 yr old male with machine injury while cutting got injured to his thumb .  Now has bone exposed over the terminal phalanx and tendon exposed and no pulp.

This case we have raised  a volar advancement flap . And as we notice its a versatile flap with both volar pedicle in the flap and inset is given with pipjt in flexion , that also is its disadvantage .Further more this flap survival is 100 %  if dissection is done close of the flexor tendons including both the artery and nerve in the flap.

Pop is to be maintained for 10 days and carry on with suture removal . Not to Forget is to give a splint post op finger straight splint and maintain for one month.
